RECEPTION
1. To enroll a child in an educational institution, parents/guardians must write:
- Application for enrolling a child An application for enrolling a child in the Public Organization "International Ukrainian School of Lithuania" (on the basis of this application, the child studies in a school in Lithuania (Vilnius, Kaunas, Klaipeda, Siauliai, Panevezys) and is entered in the register of students of Lithuania). For students in grades 1-11
- Application for determining the educational level An application for determining the child's educational level is submitted if the student lacks documents for more than the 2 previous academic years.
2. An agreement on the provision of educational services, an agreement on the provision of personal data, and on the processing of personal data are signed for the academic year
3. Together with the contract, parents submit the following documents:
- Copy of the child's birth certificate;
- Copy of the child's residence permit;
- Copy of the residence permit of one of the parents (representatives);
- Copy of the passport of one of the parents (representatives) and the child (if any);
- Document on previous education (report card, certificate of achievement, etc.);
- For grades 10-11, a copy of the Certificate of Basic Secondary Education;
- Medical certificate;
- Original personal file and original Certificate of Basic Secondary Education (for grades 10-11).
